Estimated Payment Vouchers (Form 104EP)
Used to generate Colorado estimated tax payment vouchers for taxpayers who expect to owe tax during the year and need to make estimated tax payments.
CO 204 - Underpayment Penalty
Used to calculate any penalty that may apply when sufficient Colorado estimated tax payments were not made throughout the year. For more information, please see our article here.
DR 0102 - Claim of Refund on Behalf of a Deceased Taxpayer
Used to claim a Colorado refund on behalf of a deceased taxpayer when the refund is payable to a surviving spouse, personal representative, or another eligible claimant. For more information, please see our article here.
Third Party Designee
Allows the taxpayer to authorize another person to discuss the Colorado tax return with the Colorado Department of Revenue. The designee may answer questions about the return but is not authorized to receive a refund or represent the taxpayer in legal matters.
Extension Payment for Colorado Individual Income Tax (Form DR 158-I)
Used to report a payment made with a Colorado extension of time to file.
