Colorado's Property Tax/Rent/Heat Rebate Program (PTC Rebate) provides financial assistance to certain senior citizens, surviving spouses, and individuals with disabilities who meet Colorado's eligibility requirements.
This rebate is based on property taxes paid, rent paid, and heat expenses incurred during the tax year.
Please note: This form cannot be electronically filed and must be printed and mailed when required.

Who May Qualify?
You may qualify for the PTC Rebate if you were a:
- Full-year Colorado resident; and
- Age 65 or older, or
- Surviving spouse age 58 or older, or
- Individual who meets Colorado's disability requirements.
Eligibility is subject to Colorado income limitations and program requirements.
The Following Information Will Be Needed
Qualifying Person
Select the individual claiming the rebate.
Alternate ID
Enter an Alternate ID number if required for the rebate application.
Medicaid Information
Enter the number of months during the year that Medicaid paid your Medicare premiums. If Medicaid paid Medicare premiums for all 12 months, enter 12.
Adjusted Federal SSA Benefits
Enter the amount of qualifying Social Security benefits received during the year, adjusted as required for the rebate calculation.
Old Age Pension Payments
Enter any Colorado Old Age Pension benefits received during the year.
Private or VA Pension Payments
Enter pension income received from private pension plans or Veterans Affairs benefits.
Federal Other Income
Enter any additional federal income required for the rebate calculation.
Property Tax Paid
If you owned your residence, enter the amount of property tax paid during the year.
Rent Paid
If you rented your residence, enter the amount of rent paid during the year.
Heat and Fuel Expenses
Enter qualifying heat and fuel expenses paid during the year.
Do not include heat expenses in this field if those expenses were already included in rent payments.
Meals Included in Rent
Indicate whether meals were included in your rent payments.
If meals were included for part of the year, enter the amount attributable to meals.
Tax-Exempt Housing
Indicate whether you rented a tax-exempt housing unit during any part of the year.
If so, enter the amount of rent attributable to the tax-exempt unit.
Heat Included in Rent
Indicate whether heat was included in your rent payments.
If heat was included for part of the year, enter the amount attributable to heat.
How Does This Affect My Return?
The information entered is used to determine eligibility for the Colorado Property Tax/Rent/Heat Rebate Program and to calculate any rebate amount that may be available.
This rebate is separate from the regular Colorado income tax refund.
Notes
- Property tax, rent, and heat expenses should be supported by records such as tax statements, lease agreements, receipts, or utility bills.
- Only the expenses actually paid during the tax year should be reported.
- If heat, meals, or other services are included in rent, the applicable adjustments should be made when completing the rebate application.
- Additional documentation may be required to support eligibility for the rebate program.
