Application for Extension of Time to File (Form REV-276)
Form REV-276 is used to request an extension of time to file a Pennsylvania tax return.
Important Information About Form REV-276
- Form REV-276 is not electronically filed with the Pennsylvania return.
- You must complete the form within the program.
- After completing the form, print and sign it.
- Mail the completed form to the appropriate address listed in the form instructions.
- Completing Form REV-276 does not extend the time to pay any tax due.
Taxpayers should submit any expected payment by the original due date to avoid interest and penalty charges.
Health Insurance Coverage Information Request (Form REV-1882)
Form REV-1882 may be used to request health insurance coverage information from Pennsylvania.
This form is generally used when additional health coverage information is needed for state purposes.
