The Following Information Can Be Added
Estimated Payment Vouchers (Form 1-ES)
Used to generate Massachusetts estimated tax payment vouchers for taxpayers who expect to owe tax that is not covered by withholding.
Form M-2210 - Underpayment of Estimated Income Tax
Used to determine whether a penalty applies for underpayment of estimated taxes and to calculate the amount of any penalty due.
Form M-8379 - Nondebtor Spouse Claim
Used when a joint refund may be applied to a spouse's past-due obligations. This form allows the nondebtor spouse to claim their portion of the refund.
Form M-1310 - Claim of Decedent's Refund
Used by a surviving spouse, personal representative, or other eligible claimant to request a refund owed to a deceased taxpayer.
Form M-4868 - Application for Automatic Six-Month Extension
Used to request an automatic six-month extension of time to file a Massachusetts individual income tax return.
Schedule TDS - Taxpayer Disclosure Statement
Used to disclose positions taken on a Massachusetts return that may differ from the Massachusetts Department of Revenue's interpretation of tax law.

- Filing an extension extends the time to file a return, but generally does not extend the time to pay tax due.
