IT-41 Schedule IN K-1 is used to report income, deductions, credits, withholding, and other tax information passed through to a beneficiary from an Indiana trust or estate. Information from the Schedule IN K-1 is used to properly report amounts distributed from the trust or estate on the Indiana return.

Name of Trust or Estate
The name of the trust or estate that issued the Schedule IN K-1.
Federal Identification Number
The federal employer identification number (FEIN) assigned to the trust or estate.
Foreign Address Indicator
Indicates whether the trust or estate has an address outside the United States.
Trust or Estate Address
The mailing address of the trust or estate issuing the Schedule IN K-1.
Income, Deductions, Credits, and Withholding
Information reported on the IT-41 Schedule IN K-1, including any Indiana-source income, deductions, credits, and withholding attributable to the beneficiary.
