The Miscellaneous Forms section contains additional Iowa forms that may be needed in certain tax situations. Most taxpayers will not need to complete these forms unless they are required to make estimated tax payments or need to calculate an underpayment penalty.

The Following Forms Are Available
Estimated Payment Vouchers (Form IA 1040ES)
Use Form IA 1040ES to make Iowa estimated tax payments for the current tax year. Estimated payments may be required for taxpayers who expect to owe Iowa tax and do not have enough tax withheld from wages, pensions, or other income.
Underpayment of Estimated Tax (Form IA 2210)
Use Form IA 2210 to determine whether an underpayment penalty applies due to insufficient Iowa withholding or estimated tax payments during the tax year.
Notes
- Estimated tax payments are generally made by taxpayers who expect to owe tax that is not covered by withholding.
- Form IA 2210 is used to calculate any penalty resulting from underpayment of estimated taxes.
- Not all taxpayers are required to file Form IA 2210.
