- The pass-through entity elected to pay Kentucky income tax at the entity level.
- The taxpayer is claiming a refundable credit for Kentucky Pass-Through Entity Tax paid on their behalf.

- Pass-Through Entity's Kentucky LLET Account Number

- Pass-Through Entity's ZIP Code
Kentucky Distributive Share Income
Enter the taxpayer's Kentucky Distributive Share Income Subject to Income Tax as reported on Form PTET-CR.

- Information entered in this section should match the taxpayer's Form PTET-CR.
- Separate entries may be required if the taxpayer receives Form PTET-CR from multiple pass-through entities.
- If an amended Form PTET-CR is received, indicate that the form is amended.
