Form 5498-SA reports contributions made to a Health Savings Account (HSA), Archer Medical Savings Account (MSA), or Medicare Advantage MSA (MA MSA). You may receive this form from your account trustee or administrator to show contributions made for the tax year.
If you're entering HSA information in the program, use the details from Form 5498-SA along with any information reported on your Form W-2.
Important: Form 5498-SA is generally an informational form and usually does not need to be filed with your tax return. However, the information reported on it may be needed to complete Form 8889.
Choose Your HDHP Coverage Type
When prompted, select the type of High Deductible Health Plan (HDHP) coverage you had during the tax year:
- Self-only coverage covers only you.
- Family coverage covers you and at least one other eligible individual.
Use these rules when selecting your coverage type:
- If you had self-only coverage and family coverage at different times during the year, select the coverage that was in effect for the longer period.
- If you had self-only and family coverage at the same time, you're treated as having family coverage during that period.
- If you had family coverage on December 1 of the tax year, select Family coverage.
HSA Contributions
Enter Your HSA Contributions You Made for 2026 - This amount flows to Form 8889, Line 2.
Include:
- Contributions you made directly to your HSA during 2026.
- Contributions made by someone else on your behalf.
- Contributions made by the tax filing deadline in 2027 that were specifically designated as 2026 contributions.
Do not include:
- Employer contributions.
- Contributions reported on Form W-2, Box 12, Code W.
- Rollover contributions.
- Contributions made through a cafeteria plan, since these are treated as employer contributions.
You also cannot claim HSA contributions for any month in which:
- You were enrolled in Medicare, or
- You were someone else's dependent.
Number of Months You Were an Eligible Individual
Your HSA eligibility is determined on the first day of each month.
When calculating eligible months:
- Use the family coverage limits if you or your spouse had family HDHP coverage. Ignore any self-only coverage during those same periods.
- Under the "last-month rule," if you were HSA-eligible on December 1, you may be treated as eligible for the entire year, provided you continue to meet the required testing period rules.
Archer MSA Contributions
Amount You and Your Employer Contributed to Archer MSAs for 2026 - This amount is reported on Form 8853 and may affect Form 8889 calculations.
Include:
- Contributions made by you.
- Contributions made by your employer.
- If you and your spouse had family HDHP coverage at any time during the year, include eligible contributions made to your spouse's Archer MSA as instructed by Form 8853.
Adjustment to Form W-2 Box 12, Code W
This amount flows to Form 8889, Line 9.
In most cases, the program automatically pulls employer HSA contributions reported on your Form W-2.
Do not manually enter amounts already reported in:
- Form W-2, Box 12, Code W.
You may need to complete the Employer Contribution Worksheet if:
- Employer contributions for 2026 are reported on your W-2, or
- Contributions for 2026 were made during 2027.
Keep in mind that excess employer contributions may need to be included in income.
Qualified HSA Funding Distributions From an IRA
A qualified HSA funding distribution is a direct trustee-to-trustee transfer from a traditional IRA or Roth IRA into an HSA.
This amount flows to Form 8889, Line 10.
Important rules:
- The transfer reduces the amount that can otherwise be contributed to your HSA.
- The distribution is not taxable income.
- The transfer is not deductible.
- Generally, you're allowed only one qualified HSA funding distribution during your lifetime, subject to IRS exceptions.

How to Enter Form 5498-SA in the Program
Follow these steps:
- Go to Federal.
- Select Deductions – Select My Forms.
- Choose Adjustments to Income.
- Select Health Savings Accounts (HSAs), Form 8889.
- Enter your HSA, Archer MSA, and related contribution information as prompted.
