If you've lost your W-2 form(s), or never received them, it's important to take action promptly—especially if you're preparing to file your tax return. Here's what you need to know and do:
Step 1: Contact Your Employer
Start by reaching out to the employer who issued the W-2. Request a copy of the form. Most employers can provide a duplicate if the original was lost or never received.
Step 2: If Your Employer Cannot Provide the W-2
If your employer is unable to provide the form, you’ll need to contact the IRS directly. Be prepared to provide the following information:

Your personal details:
- Full name
- Mailing address
- Phone number
- Taxpayer Identification Number (e.g., SSN or ITIN)
- Dates of employment with the employer

- Name
- Mailing address
- Phone number
- Employer Identification Number (EIN), if known
The IRS will then contact your employer to request the missing W-2. You will also receive:
- Form 4852 (Substitute for Form W-2)
- A letter with instructions on how to complete and use Form 4852
Step 3: Filing Without Your W-2
If you still haven’t received your W-2 by the tax filing deadline:
- Use Form 4852 to estimate your wages and tax withholdings.
