If you received one or more Form 1099-LTC forms for the insured and Box 3 is checked "Per Diem," enter the total amount shown in Box 1 from all of those forms.
The IRS defines per diem payments as payments made at a fixed daily, weekly, or monthly rate, regardless of the actual long-term care expenses incurred.
How to find the amount
- Review each Form 1099-LTC received for the insured.
- Check Box 3 to see whether "Per Diem" is marked.
- For every form with Per Diem checked, locate the amount in Box 1.
- Add together the Box 1 amounts from all qualifying forms.
- Enter the total as your Gross LTC premiums received on a per diem basis.
