When entering the School District information from your W-2 form, Ohio requires you to enter the school district code instead of the name on your W-2 Form.
To successfully enter this within your account, you will make two entries under State and Local Information. You will enter your Ohio state information with your county information in the "first" state entry section. Then, you will click the green plus sign to create an additional or "second" state entry section. In the second State and Local Information section, you will select the name of the state (Ohio) and enter the state ID, but this time you will leave the state wages and state income taxes blank and make the entries for the school district information using boxes 18, 19, and 20. Enter the school district tax in box 19. This may be indicated on your W-2 by the letters, SD. Box 20 needs to include the school district code (not the name). The school district has to be the second state entry on the W-2 or it will not pull to the SD-100.

If you do not have a separate Locality or County line, enter the school district 4-number code information in box 20.
If you have more than one County line, you will need to enter that on a separate W-2 Form within your account.
